Appeals and Refund Address Change:

This notification is to inform providers of an important change regarding two mailing addresses. The mailing address for submitting provider claim appeals and refund request was updated. If your claim appeal was affected by the address change, providers can resubmit the appeal to the correct address with verification to support the filing.

Effective immediately, all appeals should be sent to the following new address:

New mailing address for claim appeals:

As always, provider claim appeals can be submitted through the MHS online secured portal. Please contact your personal Provider Engagement Administrator for more information or training on the MHS secured portal.

New mailing address for Refunds:

Carol Stream, IL 60132-3656

These address changes are effective as of the date of this letter. We request providers update their records and ensure all future appeals and refunds are directed to the new address.
