Hoosier Care Connect

Hoosier Care Connect is a health care program for individuals aged 59 years and younger, blind or disabled and who are also not eligible for Medicare. 

Hoosier Care Connect covers a variety of individuals who are not eligible for Medicare, including: 

  • Blind individuals
  • Disabled individuals
  • Individuals receiving Supplemental Security Income (SSI)
  • Individuals enrolled through M.E.D. Works
  • Some foster care children 

Members of the Hoosier Care Connect plan will select a managed care entity (MCE) responsible for coordinating care in partnership with their medical provider(s). MHS is an MCE for Hoosier Care Connect. 

Hoosier Care Connect members will receive all Indiana Medicaid-covered benefits in addition to care coordination services. Care coordination services will be individualized based on a member’s assessed level of need determined through a health screening. 

All services must be medically necessary. Your Primary Care Provider will work with you to make sure you get the services you need. These services must be given by your Primary Care Provider or by another provider that your Primary Care Provider refers you to. 

Some services may:

  • Have coverage limits.
  • Need a doctor’s order.
  • Need prior approval.

*Some Medicaid members may not have all the benefits listed.
