Disease Management

MHS provides services for many conditions, such as asthma, diabetes, COPD, high-risk pregnancy and many more. We also offer a Children with Special Needs program for children with developmental or chronic health conditions.

If you have, or are at risk for having, one of the following health conditions listed, please call so that we can tell you more about and offer to enroll you in a program. If you would like to find out more, or to join a program, please contact us.

Specialized Health Programs

  • Asthma
Asthma cannot be cured, but most people with asthma can control their symptoms and prevent asthma attacks by avoiding asthma triggers and correctly using prescribed medicine, such as inhaled corticosteroids.
  • Attention-Deficit / Hyperactivity Disorder (ADHD)
Children with ADHD have trouble paying attention, controlling impulsive behaviors (may act without thinking about what the result will be), and in some cases, are overly active.
  • Bipolar Disorder
Bipolar disorder, also known as manic-depressive illness, is a brain disorder that causes unusual shifts in mood, energy, activity levels, and the ability to carry out day-to-day tasks.
  • Chronic kidney failure (CKD)
CKD is a disease where the kidneys become less able to clean waste and extra fluid out of the blood.
  • Chronic obstructive pulmonary disease (COPD)
COPD is a group of lung diseases that cause you to have trouble breathing.
  • Congestive heart failure (CHF)
CHF is a disease that occurs when your heart is too weak to pump blood.
  • Coronary artery disease (CAD)
CAD happens when a substance called plaque builds up in the arteries that supply blood to the heart (called coronary arteries).
  • Depression
Depression is a mental illness that affects your thoughts and feelings. It can cause problems in your everyday life. It can make you feel sad or down for no reason. People who have family members with depression are more likely to have it too.
  • Diabetes (Sugar)
Diabetes is also known as “sugar”. Diabetes type 1 is a disease where your body can no longer make insulin. Diabetes type 2 is a disease where your body cells are less sensitive to your insulin. Both types can cause high blood sugar levels. MHS Diabetes Education Program (PDF)
  • Hepatitis C
Hepatitis C is a virus that attacks the liver. Many people show no signs of an infection, but those who do may experience fatigue, nausea, or a lack of appetite.
  • HIV
HIV is an acronym for the human immunodeficiency virus. This virus can be spread through unprotected sexual contact, blood transfusions, or sharing contaminated needles.
  • Stop Smoking
MHS encourages you to quit using tobacco products or smoking today. Tobacco smoke contains nicotine, a drug that is addictive and can make it very hard, but not impossible, to quit. More than 400,000 deaths in the U.S. each year are from the use of tobacco products.

Care Management

The care management level of service is for members who need help with care coordination, making preventive care appointments, or accessing care for a chronic health condition. Care management can also help with social determinants of health issues like food, transportation and housing.

Complex Case Management

The purpose of complex case management is to support recovery and resiliency, provide support during transitions of care, and to improve treatment adherence and outcomes. MHS staff work with members who have complex, chronic and/or behavioral health conditions including substance use disorder that require coordination of services and periodic monitoring.
