Managed Health Services (MHS) is proud to offer community-based organizations and providers the opportunity to apply for grants to improve accessibility around their offices. The grant can be used for building a wheelchair ramp, toilet stall retrofit or Compassion Closet, in accordance with Americans with Disabilities Act (ADA) standards.
Who is eligible: | Community-Based Organizations (CBO) in the State of Indiana that work specifically with Aged, Blind or Disabled (ABD) populations (i.e. Bosma, Blind Social Center, Area
Agencies on Aging, etc.) |
What can be requested through the grant: | - ADA-compliant aluminum access ramp
- Bathroom retrofit to ADA compliance; one stall
- Compassion Closets

What is requested of the prospective CBO: | - Scope of why they want/need the grant
- Scope of what is needed (ramp installation, stall remodel, etc.)
- Number of clients that come to their facility each week/month/year
- Photo of where they want the ramp installed or remodel to take place
- Any other information you want us to know
- Budget proposal

Criteria applicants will be judged on: | - Amount of MHS members in the same zip code of the CBO
- 145 points available
- An organization will receive:
- 20 points for serving 100+ MHS Hoosier Care Connect (HCC) Members
- 10 points for serving 50-99 MHS HCC Members
- 5 points for serving 0-49 MHS HCC Members
- 25 points for being located in a county with 5,000 + MHS Hoosier Care Connect Members
- 15 points if the organization offers emergency housing
- 15 points if the organization has a food pantry onsite
- 15 points if the organization has partnered with MHS for an event in the past 24 months
- 15 points if the organization serves the Aged, Blind or Disabled population in some form
- 25 points if the organization is contracted as an MHS Provider
- 15 if the organization is a Minority/Women-owned Business Enterprise (MWBE)

Frequency of grant allotment by the Health Plan: | - September 27, 2024
- All applications will be reviewed by October 21, 2024.
- All four organizations that will receive a grant for the calendar year 2024 will be selected and notified by October 31, 2024.
- The installations will take place by March 31, 2025.

Other Details:
| - Must be a community-based organization in the State of Indiana.
- Program begins September 2, 2024 at 12 a.m. Eastern Standard Time (ET).
- Employees, officers and directors of Managed Health Services (MHS) (“Health Plan”), its affiliates, subsidiaries, and members of their immediate family are not eligible.
- VOID OUTSIDE OF THE ELIGIBILITY AREA AND WHERE PROHIBITED OR RESTRICTED BY LAW.
- There will be one remodel or installation awarded each quarter (installation of a wheelchair ramp or remodel of a bathroom stall so that it is wheelchair assessable).
- All four organizations that will receive a grant for the calendar year 2024 will be selected and notified by October 31, 2024.
- Recipients may not transfer grant to another organization.
- Recipients may not substitute grant for other items.
- Dollar value of each grant will vary depending on location, space, choice of remodel, or wheelchair ramp and supplies.
- Odds of being selected as a grant recipient depend on total number of eligible entries submitted.
- Organizations selected as grant recipients will be notified by phone or email. (These organizations will be required to respond within seventy-two (72) hours of the initial notification or the grant will be awarded to another organization.)
- Ramps or remodels will be installed by coordination between the Health Plan and the contracted vendor.
- The installations will be completed no later than March 31, 2025.
- Acceptance of grant constitutes permission to use winners name and/or likeness for purposes of advertising and promotion without further compensation in all media now known or here after discovered worldwide without notice or review or approval, except as prohibited by law.
- By accepting the grant, the CBO agrees to release and hold the Health Plan harmless from all losses, damages, rights, claims, and actions of any kind (including, without limitation, personal injuries, death and property damage) resulting from acceptance, possession, or use/misuse/nonuse of any ramp or remodel.
